I don't have any names, this is the non identifying information I recv'd form the state when I was 18. BIRTH MOTHER Age:20 Nationality: German, English, Irish Religion: Protestant Height: 5’ 3” Weight: 120 lbs Hair: Long Blonde Eyes: Green Complexion: Fair Bone Structure: Slender Education: Completed GED Marriage Status: Married Occupation: Factory assembly line supervisor Personal Characteristics: NI Interests/Hobbies/Talents: Art, obtained training as a paramedical technician. Medical Genetic Background: No problems noted. Birth mother’s other children and Description: Girl at age 2 at time you were born. She had hazel eyes, light brown hair, fair complexion. A brother born in 1972 was also relinquished for adoption. You had an older brother who was deceased. Reason for Relinquishment/Termination: because of family problems it was felt that relinquishment was best for you. This was a difficult decision for your birth mother. Additional information: Your father was responsible for the death of one of your siblings. He was of German, English and Irish Nationality. He had completed the 8th grade and was employed as a laborer. His health was reported to be good. Maternal Grandmother: Age 42, 140# 5’5” with light green eyes, light brown hair, and a fair complexion. She was of German Decent. She had completed some college and was a secretary. She enjoyed clubs and bowling. She suffered from asthma and allergies. Were Maternal Grandparents aware of birth/pregnancy: NI Other Maternal Relatives: Your birth mother had a twin sister. She had an older sister age 22 and an older brother age 21. Her sisters completed high school and her brother the 11th grade. One sister worked in a factory and one was a housewife. Her brother worked in construction. Her twin sister had a low normal I.Q. No health problems were for the sibs. PATERNAL RELATIVES Paternal Grandfather: Deceased at age 56 from heart attack. Other birth record information: 40 weeks gestation, mild physiological jaundice resolved at discharge prognosis was excellent. Labor 8 hrs. Birth was easy. Subsequent medical exams: At 5 ½ months you weighed 15 pounds 4 ounces, length 25 ¼”, head 17 ¼”, chest 16 ¼”. You are described as a happy , alert responsive infant wh performed well and no evidence of structural abnormalities. Findings were within normal limits. 10 months, weight 17 pounds 5 1.4 ounces, length 27 ¼”. You were able to get into a sitting position alone, crawled well, said “mama, dada”, patty caked and performed up to expectancy. Attractive well nourished, very limber infant who sat well, reached accurately and banged blocks. 0/2 teeth. Foster Care Placement: You were placed from the hospital at 5 days. You had an pronounced startle reaction which lessened but still persisted. You would start to cry at unusual sound ( a cough/ sneeze ). You had a history of diarrhea and were susceptible to diaper rash.
