ISO Birth mother and/or siblings. I have an "amended" birth certificate so am not certain of any of the info on it. Says I was born Feb 14, 1945 in Cushing, Ok Payne Cnty, at 10:15AM. The attending physician's name is signed E.O. Martin, MD who I understand died sometime before 2002. My adopted parents names and info is stated instead of birth parent info. I was adopted in Independence or Liberty, Mo. in 1948. My adopted parents petitioned to change my birth name but the Judge rejected that request. My birth name is Theodora.
